The three bills: consultant, anaesthetist and hospital fees explained

The most common surprise in UK self-pay surgery is not the total. It is the number of envelopes. Understanding why there are three makes every quote you receive easier to read.

Why three bills, and not one
The Private Healthcare Information Network (PHIN), the government-mandated body that publishes information on private healthcare, puts it plainly: "You will usually receive three bills: one from the hospital, one from your doctor, and one from the anaesthetist." The reason is structural. Your surgeon and anaesthetist frequently do not work for the hospital. As PHIN describes it, "they are independent professionals who use its facilities to treat you", so each can invoice you directly, while the hospital bills separately for your room, nursing care and operating theatre use (PHIN, updated 16 June 2026).
What each bill covers
- The consultant's bill
- The surgeon's time, skill and clinical responsibility. It normally starts with the outpatient consultation, which PHIN describes as a distinct, one-off cost paid on the day and "almost never included in the cost of subsequent surgery, even within a fixed-price 'package deal'". PHIN puts the consultation fee at typically between £150 and £300, and says you should get the exact fee in writing in advance.
- The anaesthetist's bill
- A second consultant, usually independent of both the surgeon and the hospital, responsible for anaesthesia and your safety during the procedure. In a package this is bundled in. Outside one, it arrives on its own.
- The hospital's bill
- Theatre time, your bed, nursing care, standard drugs and consumables. This is the part that most obviously varies by where you have the procedure, because it reflects a building, a staff rota and a local market.
The one question that collapses the confusion
PHIN suggests asking upfront: "Will all fees be combined into a single bill, or should I expect separate invoices?" Ask it before you agree to anything, and ask for the answer in writing. A hospital that quotes a genuine all-in package will say so without hesitation, and a hospital that bills itemised will tell you which invoices to expect.
Where the money actually differs
Because the hospital component is the largest and the most local, the same operation costs materially different amounts in different cities. PHIN publishes average self-pay package prices by location, taken from publicly available data collected in March 2026. For hip replacement, its city averages run from £13,847 in Glasgow to £17,540 in Cambridge, with London at £15,712 (PHIN average package prices). PHIN describes these as a guide only, and notes prices may have changed since collection.

Hip replacement is used here only because PHIN publishes comparable regional figures for it. It illustrates the size of the location effect, roughly a quarter between the cheapest and dearest city average in that dataset. Do not read across to your own procedure. What this means when you compare quotes
- Compare like with like. A lower package price that excludes the anaesthetist is not a lower price.
- Add the consultation yourself. It sits outside almost every package, so put it back into your own total before comparing.
- Ask about diagnostics separately. PHIN notes that diagnostics and pathology costs "in particular often catch patients out and can contribute significantly to bills".
- Get it in writing. PHIN's summary advice is to always ask for a written quotation and check with your insurer, if you have one, before you agree to anything.
